If you have a student that attends a school that is Out of Zone(OOZ), you'll be happy to know the Calcasieu Parish School Board (CPSB) has the new 22-2023 school year applications available. Parents/guardians may pick up the OOZ apps now at the Child Welfare & Attendance Office, located at 2423 6th Street in Lake Charles. Office hours of operation are 8 am-11:30 am and 1 pm-3:30 pmApplications can also be obtained by email request if you are unable to visit the office in person, at cwainfo@cpsb.org.

If you are requesting an OOZ app via email, please include your name, mailing address, and the number of forms being requested. OOZ apps WILL NOT be distributed or accepted at CPSB schools. The deadline to return OOZ apps is Tuesday, May 31, 2022, and they must be mailed or hand-delivered back to the Lake Charles Child Welfare and Attendance Office. Some required items to complete the application is as follows:

1.) Guardian’s photo id
2.) Current electricity statement with the service address listed
3.) Another current utility statement with the service address listed. Examples: water, gas, and cable. A lease containing the guardian as the lessee and the signature of the leasor is also accepted.
4.) Birth certificate
5.) Other legal documents, if applicable

Please note: You will need to make your own copies of documents requested on OOZ apps and turn them with the completed application. The Child Welfare and Attendance Office will not make copies of any required documents for the application. Parents/guardians will get the results of OOZ requests via USPS in mid-July.
